FIRE Program Commitment Agreement

Commencing the week of Monday, 13 October 2025

By clicking “Submit,” I confirm my enrollment in the upcoming FIRE Program at Creighton Personal Training and agree to the following terms:

Program Enrollment

I am securing a place in the 10-week FIRE Program, commencing the week of Monday, 13 October 2025.

Payment Terms

I agree to pay the full program fee of $785 via direct debit in weekly instalments of $65, beginning the week of 13 October 2025, from my nominated account.
Although the FIRE Program runs for 10 weeks, payments are spread over 12 weeks for the convenience of cash flow.
The full amount remains payable regardless of attendance.

Withdrawal Prior to Start

If I withdraw before the program begins, I am liable for a non-refundable amount of $350, which will be debited from my nominated account in weekly instalments.

Withdrawal After Commencement

If I am unable to complete the program due to illness, injury, or any other reason, I acknowledge that I remain liable for the full $785, which will continue to be debited in weekly instalments from my nominated account.

Attendance & Make-Up Sessions

The FIRE Program includes two face-to-face training sessions per week at my scheduled time.
If I miss a session, I may complete a solo make-up session on Saturday mornings at 7:00 a.m. (trainer not present).

Participant Responsibility

I understand that it is my responsibility to attend scheduled sessions and participate in a manner consistent with my current health and physical abilities.
I agree to notify Creighton Personal Training of any relevant medical conditions or injuries that may affect my participation.

Waiver of Liability

I acknowledge that participation in the FIRE Program involves physical activity and carries inherent risks.
By agreeing to this commitment, I release and discharge Creighton Personal Training and its staff from any liability for injury, loss, or damage sustained during participation, to the extent permitted by law.

Non-Transferability

This agreement is non-transferable. Program enrollment and payments may not be assigned or shared with another individual.

Privacy & Data Consent

I consent to the collection and secure storage of my personal information as required to process this agreement and deliver program services, in accordance with Australian privacy law.
